Transitional Care Centre

Caledon Community Services' 24/7, 365 days a year short-term Transitional Care Centre is a compassionate and innovative approach to caring for our community's ill seniors. Aimed at freeing up acute care and emergency department beds in hospitals, it promotes the use of community care services while also helping to ease the many stressors facing caregivers and families of our aging senior population. The Centre is staffed by Personal Support Workers, Respite Workers and many of our devoted Caledon community volunteers. It is located within the Supportive Housing Building of Maple Grove in quaint Caledon Village.

Personal Support Workers (PSWs) and Respite Workers work within a cozy two-bed facility to provide individualized attention and intensive support with all activities of daily living including bathing, dressing, medication reminders and nutritious meals and snacks catered to each client’s dietary needs and restrictions.

The Transitional Care Centre staff are committed to engaging their residents with on-site activities and through individual care and conversation aimed at promoting an increased sense of self worth and community involvement. Our direct services are complimented by case management, referrals and transportation services, all in an effort to preserve clients' well-being.

The multi-disciplinary team provides comments and observations to the Centre’s Care Coordinator who regularly communicates with the Community Care Access Centre’s Case Managers and the hospitals' Discharge Planners.

Who can Access the Care Centre?

The Transitional Care Centre is accessible to seniors residing within the Municipality of Caledon and Central West Ontario who are in need of short-stay respite services. The Centre was developed to meet the needs of persons who have completed their acute care phase in hospital and are in need of more intensive PSW/Respite support services before returning home.

Is There a Fee connected with the Centre?

Clients will be required to pay a fee of $25/day; however, financial assistance may be arranged for low income clients and families.
Clients will be offered the opportunity to participate in community-based Adult Day Programming (if appropriate). Transportation for outings i.e. doctor appointments, day programs, shopping, etc. can be provided through CCS’ Transportation Program at an additional charge to the client.

How Long can I Stay at the Centre?

Clients can stay at the Centre for up to 7 days. In extenuating circumstances, that period may be extended to a maximum of 14 days.

Is the Centre Co-ed?

The Transitional Care Centre consists of a semi private bedroom. Although both men and women are accepted into the Centre, we will only accept the same gender at any given time.

How do I Make a Referral to the Centre?

You can contact the Care Centre Coordinator to begin a referral process at 905.584.2300, ext. 663. The Coordinator will then go out to the hospital or potential client's home to conduct an assessment in order to determine if the Transitional Care Centre is a suitable care setting at this time.
